Updated Python versions

Select the correct SDK for github CI and azure pipelines

Use latest Xcode version

Allow any Xcode 12 version

Update latest PyPy version

MRG: Refactor pip install to work round end of pip support

Move pip install commands into their respective platform / Python installs, for clarity.

Use `ensurepip` where possible.

Work around end of pip support for Pythons 2.7 and 3.5.

Deprecate install_pip function as confusing.  Move pip checks into
functions for individual platforms.

MRG: set MACOSX_DEPLOYMENT_TARGET for arm64 build

MRG: Support building arm64 and universal2 wheels on x86_64

Add `arm64` and `universal2` PLAT options.  `universal2` works by seperate Intel and ARM64 builds, and fusing the resulting wheels.
